It is with mixed emotions that I announce the closure of Today Integrative Health + Wellness as you have known it. I have truly enjoyed working with the exceptional team of providers and staff that were in our Lake Oswego Location.
The last day of clinical operations at the Lake Oswego location was September 28, 2018.
The former clinical team of Today will continue to practice in new locations and they are excited to continue care with you. Please check the provider pages on this website for practice updates.
My new practice location has been added to that page and you may begin scheduling at the new location.
The main clinic phone number is still active and you may leave a secure voice message there. Those message will be checked once daily.
After the clinic closure, we will continue to be stewards of your medical records. Should you want another provider to have a copy of your medical records, you may fill out an authorization form and fax or mail it in. The authorization forms may be found on our website under the section for patient forms
